Do you need to change your Illinois Medicaid Health Plan?

To change your Illinois Medicaid plan, you can:

1. Change your health plan one time in the first 90 days¹.
2. Call the State’s Client Enrollment Services at 877.912.8880 or visit their enrollment website².
3. Call the Client Enrollment Broker Call Center at 1-877-912-8880 or go online to the Enrollment Portal at www.enrollhfs.illinois.gov to get more information about your HealthChoice Illinois plan choices and to make a plan switch³.
